The Ohio FAIR Plan does not offer paper versions of the applications for:

• Dwelling Fire
• Dwelling Fire Rehabilitation
• Homeowners
• Commercial
• Commercial Rehabilitation
• Farm

Instead, please log in to submit an electronic application. If you do not yet have a login, you can request one. If you have questions about the electronic application process, please refer to our FAQs.

OFPUA does still accept paper applications for:

• Commercial Crime
• Residential Crime
